Are drug-eluting stents cost-effective? It depends on whom you ask.

چکیده
Since their commercial introduction in 2003, drugeluting stents (DES) have rapidly altered the management of coronary artery disease. Before their development, the percutaneous management of coronary artery disease was performed predominantly by implantation of bare metal stents (BMS) made of either surgical stainless steel or metal alloys. Although such stents represented a considerable advance over balloon angioplasty alone, they remained limited by restenosis resulting from neointimal proliferation. As a result, 15% to 20% of patients treated with BMS required 1 repeat revascularization procedure within the 6 to 12 months after stent implantation.1 Despite numerous attempts at systemic pharmacotherapy, device modification, and even use of ionizing radiation, the rate of restenosis after BMS implantation remained largely unaffected.

Arterial drug concentration distribution determines local toxicity. The safety issues dealt with Drug-Eluting Stents (DESs) reveal the needs for investigation about the effective factors contributing to fluctuations in arterial drug uptake. The current study focused on the importance of hypertension as an important and controversial risk factor among researchers on the efficacy of Heparin-Eluti...

BACKGROUND The value of drug-eluting stents in preventing cardiovascular events has not been investigated in Mexico. OBJECTIVE To conduct a cost-effectiveness analysis of early and new-generation drug-eluting stents from the perspective of a healthcare provider. METHODS We conducted a cost-effectiveness analysis of early and new-generation drug-eluting stents in patients with ischemic cardi...

In their recent systematic review, Suzanne Ligthart and associates compared analyses of the cost-effectiveness of drug-eluting stents. They found that in most studies in which an incremental cost-effectiveness ratio greater than $50 000 per quality-adjusted life-year was calculated the study authors recommended against the widespread use of drug-eluting stents. However, we believe that previous...

OBJECTIVE To determine the cost-effectiveness of drug-eluting stents (DES) compared with bare-metal stents (BMS) in patients requiring a percutaneous coronary intervention in France, using a recent meta-analysis including second-generation DES. METHODS A cost-effectiveness analysis was performed in the French National Health Insurance setting. Effectiveness settings were taken from a meta-ana...
